From X-Ray to Evidence: The DXD Workflow
Standard imaging can miss the real cause of pain, especially when the injury involves movement, not structure. That’s where DXD comes in. Our system analyzes digital motion X-rays to detect spinal instability caused by damaged ligaments.
Using AMA impairment guidelines and proprietary measurement tools, we identify abnormal vertebral motion and provide clear, annotated reports that support diagnosis, documentation, and legal claims. Here’s how it works.
Static Scans Miss What Motion Reveals
Traditional MRI and X-ray capture the spine at rest, but ligament injuries often only show up when the spine moves. That’s why DXD uses digital motion X-rays to reveal abnormal movement patterns that correlate with pain and instability.
Our system measures vertebral translation and angular rotation between flexion and extension, comparing those values against established clinical thresholds. This dynamic view helps identify true functional injuries that static imaging can overlook.

A Validated Method for Measuring Spinal Instability
AOMSI stands for Alteration of Motion Segment Integrity. It’s a method defined in the AMA Guides to the Evaluation of Permanent Impairment that uses motion-based thresholds to determine whether a spinal segment is unstable.
Instability is quantified by measuring:
Translation (how far one vertebra slides forward/backward relative to the one below)
Angular rotation (how much the segment rotates during motion)
These values are compared against established limits:
> 3.5 mm of translation (cervical)
> 11 degrees of angular rotation between segments
If thresholds are exceeded, the segment is considered unstable — even if static imaging looks normal.
From Motion Data to Admissible Impairment Ratings
If motion findings meet or exceed AMA thresholds, DXD applies formal impairment language from the 4th, 5th, or 6th Editions of the AMA Guides. The report will include:
A clear statement of impairment
Edition-specific citations
Segment-level breakdown of motion findings
Annotated images showing threshold exceedance
These ratings are used in both clinical documentation and legal cases to demonstrate permanent injury, support claims, and justify treatment plans.

About Dr. Cielo
Dr. Todd Cielo is the founder of DXD and a practicing chiropractor with over 20 years of experience in spinal injury care, education, and interdisciplinary collaboration. He is a lecturer for continuing education programs across Florida and the accredited CLE speaker for the Florida Bar Association. Appointed by the Governor to the Florida Board of Chiropractic Medicine, Dr. Cielo brings deep clinical and medico-legal insight to every DXD report. His mission is simple: to bring objectivity, credibility, and better outcomes to soft-tissue injury diagnosis.
